What are the call centre opening times?

The call centre staff can be contacted by telephone from 08.00 to 20.00 Monday to Friday and 09.00 to 13.00 on Saturday.

Multi language support is available for patients.

 

Our web site is available 24/7 and has a wealth of information about diabetes and patients can book a course directly.

 

Who is eligible to be referred?

Anyone with Type 1 or Type 2 diabetes and registered with a GP practice within the Kent and Medway CCG area, including those of no fixed abode.  Patients being referred to the service should be 18 or over.   

The Hub is for people who have received a diabetes diagnosis (Type 1 or Type 2) and would benefit from structured education and those with Non-Diabetic Hyperglycaemia eligible for the National Diabetes Prevention Programme. All people with diabetes must be registered with GPs in one of the listed Kent and Medway CCGs.

The Hub is not just for people that are newly diagnosed, anyone who has been diagnosed with diabetes would be able to use the Hub to get access to Structured Education. We know that many people have had diabetes for many years and have never attended a course, we are keen that they are encouraged to attend, and HCPs will be encouraged to also refer these people.

 

Are there any exclusions?

Initially type 1 self-referral will be excluded as there are complexities in the pathway that need to be resolved.  We envisage that Type 1 referrals will be included after approximately the first six months of the operation of the Hub.
